We heard the need to identify a reason why a patient has been redirected to another facility.
Context
The new Facility Redirect Reason feature allows Patient placement specialists using the Transfer IQ® application to indicate the reason a patient was redirected to a facility different from the preferred facility.
Use Cases
Transfer IQ®
Referring Facility calls (or electronically submits via Community Access® portal) the Transfer Center indicating a patient needs to be transferred and they indicate a preference of Campus A.
Campus A is at 100% capacity and is on diversion, so, therefore, cannot accept any more patients.
However, Campus B has capacity to accept the patient.
The TransferCenter agent indicates that Campus A will redirect the patient to Campus B, but is unable to record the reason in a field that is reportable.
Data IQ®
As a TransferCenter Manager or Director, I would like to see how many times patients were redirected to a facility that was different than the initially indicated Preferred Facility and the reason they were redirected.
This information is important to understand to ensure patients are redirected appropriately. It can also identify instances where the Transfer Center staff is consciously taking pressure off the main "hub" hospital and offloading less acute patients to a community hospital to help balance patient load. Another example might bring to light situations where patients are being redirected to another facility within the health system due to resource or equipment deficiencies. This type of data can help our customers understand where needs/deficiencies exist.
Functionality
Selecting a Redirect Disposition on a Facility Card will open a prompt for a Redirect Reason to be selected.
A single Redirect Reason can be selected from the dropdown.
Additional Redirect Reasons can be added in the disposition reason dictionary.
A new Facility Redirect Column has been added to the disposition reason dictionary.
Any dictionary entry associated with the facility redirect column will appear in the list of redirect reasons.
Add Reason Workflow
Select the Facility Redirected icon from the Target Facility Communication section on the case’s Communication tab.
A window will appear prompting for a Redirect Reason. Choose a reason from the list and select Redirect Reason.
The window will close and the Redirect Reason will be added to the Case Log.
Dictionary
Refer to Create Dictionary Items on how to adjust dictionary items.
Case Log
Reasons appear in the case log underneath the facility selection.
Columns
Logic has been updated for the Redirect Reason Column on the Case List
Prior to this update a Declined Reason would update the Redirect Reason Column. It no longer updates the Case List
When a redirect reason is selected it now populated the Redirect Reason Column
The most recent Redirect Reason will populate the column if there are multiple selected.
Reporting
Currently, the Facility Redirect Reason is not available within Data IQ®.
This will be available at a later date.
