Introduction
Discharge Disposition is a key piece of patient information that indicates where a patient will go after leaving the hospital, such as home, a rehab center, or a skilled nursing facility. This value plays a vital role in discharge planning, helping care teams ensure safe and timely patient transitions. It also supports operational insights, such as discharge trends and time-to-leave metrics. The field is managed via a new dictionary in the Admin Tool and can be configured, displayed, and filtered in alignment with your organization’s workflows and integration needs.
Function
Discharge Disposition is managed through a new dictionary in the Admin Tool. Users can:
Add new entries
Deactivate or Activate existing entries
Save changes
Delete entries (with confirmation)
Go to the Dictionary Management > Discharge Disposition list
Select the check box next to the row for the discharge disposition that you want to activate or deactivate.
Select Activate to enable the discharge disposition.
The discharge dispositions row becomes enabled, and you can select it.
Select Deactivate to disable the discharge disposition.
The discharge disposition row turns gray and becomes disabled
To add, edit, or delete a discharge disposition:
Do one of the following to display the Dictionary Management page.
Go to Admin Tool > Dictionary Management
Go to Admin > Settings > Capacity management > Miscellaneous > Discharge Disposition
In the Dictionary Name Column, click the Discharge Disposition link
To add discharge dispositions:
Click Add to add a new row at the bottom of the page
In a blank row in the Name column, type a discharge disposition (for example: Home, Rehab Facility, Skilled Nursing)
In the same row for the Display icon, select the image that will be used to identify this discharge disposition on the console. Click the magnifying glass icon to display a window that shows a list of available images. Click to select one of the images, and then click Ok
To remove an image that has already been selected, click the x in the upper-right corner of the picture
Click Save
The new discharge dispositions repeat the step above
To add more discharge dispositions, in the appropriate rows and columns, change the information, and then click Save
To delete a discharge disposition:
You cannot delete a discharge disposition that is already associated with patients.
Do one of the following
To delete all discharge dispositions, select the check box in the first row and first column of the page
To delete individual discharge dispositions, select the check boxes for them.
Click Delete Selected Records
When a user deletes an entry from the Discharge Disposition dictionary, a confirmation message is displayed to confirm the action and prevent accidental deletion.
Dictionary
Discharge Disposition is configured in the Admin Tool under a new dictionary located between Diet and Discipline.
Dictionary fields include:
Name: Maximum 200 characters
ADTID: ADTID is a Required Field. Each Discharge Disposition entry must include an ADTID. This is a required field and follows the same structure as other dictionary entries. It ensures correct mapping from HL7 messages to display values.
Icon: Optional, selectable like other icon-enabled dictionary items
The Name column supports alphabetical sorting to aid usability.
Available in PreadmitTracking or PatientTracking Portal
Discharge Disposition is displayed in both:
PatientTracking Portal (Portal): as a read-only UI element with an optional icon and name
PreAdmitTracking Console (PAT): as a text-only, read-only column
Discharge
This field is used to indicate a patient’s discharge destination (e.g., “Patient going home,” “Rehab,” “Skilled Nursing”). It provides critical insight for discharge planning and helps care teams align resources and expectations for post-discharge care.
Filters
This field can be used as an advanced filter within supported views, allowing users to filter patients by discharge type (e.g., only show patients going to Skilled Nursing).
Integration
The Discharge Disposition value is populated via HL7 transactions using the PV1.36 segment. It is:
Automatically consumed into Capacity IQ
Updated when transactions change (added, modified, or removed)
Nulled out when an empty quote is received
The field is read-only for users and must be mapped properly by the integration team to values defined in the dictionary.
Disposition Integration
With the 23.12 release, Discharge Disposition is available as a read-only field in both the PatientTracking Portal and PreAdmitTracking Consoles (PAT), populated automatically through HL7 transactions. It enhances discharge planning, enables reporting and filtering by disposition type, and improves the accuracy of patient tracking data. Organizations should coordinate with their TeleTracking representative to ensure proper setup, integration, and value mapping.
