Typical workflow of a Patient Placement Specialist

The Transfer IQ® application tracks and manages each patient transfer from the initial telephone transfer request, until the case is closed. Each patient transfer is issued a unique transfer (case) number and the transfer case becomes a permanent record. This workflow covers a typical application workflow mainly used by Patient Placement Specialists.


Steps

  1. The transfer center receives a phone call or other communication requesting a patient transfer from a referring facility.

  2. The patient placement specialist opens a new transfer case and begins collecting information about the patient, the referring facility, and the referring physician.

  3. The patient placement specialist contacts the on-call physician for the requested specialty to possibly consult on the transfer case.

  4. The patient placement specialist conducts a conference call with the referring physician and the on-call physician to determine if the destination facility will accept the patient.

  5. If the facility accepts the patient, the on-call physician becomes the accepting physician and is responsible for the patient's care during the on-call physician's shift.

  6. The patient placement specialist dispatches a transport company to pick up the patient if required.

  7. The patient placement specialist notes the transfer patient's arrival time at the destination facility.

  8. The patient placement specialist makes a bed request for the transfer patient and notes the time that the bed is assigned.
