About

The Referring Information section contains fields to populate information regarding Referring Facilities. One of the first sections that Patient Placement Specialists will fill out.


Steps

We’ll review each sub section and how they’re used.


Referring Facility

Fields covering Facility Name, Unit, and Bed Number of patient.

  • Facility

    • Begin typing the name of the patient facility and select the appropriate facility.

    • If you need more information select the blue Information icon.

  • Unit

    • Begin typing the name of the patient facility and select the appropriate Unit.

  • Bed #

    • Enter the identifier for the patients bed from the Referring Facility.


Referring Physician

Information about the physician and their contact number.

  • Name

    • Begin typing the name of the patient facility and select the physician from the list.

    • If you need more information select the blue Information icon.

    • If you do not find the appropriate physician, and are integrated to the NPI registry, utilize the Search for New Physician Functionality

  • Phone #

    • Number associated with the physician automatically populates.

  • Ext

    • Will auto populate if there is an associated extension.


Diagnosis

Nature of the patients disease/injury.

  • Primary Diagnosis

    • Primary reason for medical attention

    • Begin typing and select the appropriate diagnosis from the list.

  • Secondary Diagnosis

    • Begin typing and select the appropriate diagnosis from the list.

  • Protocol

    • Type to auto populate protocols.

    • Once you have one selected you can click on View Protocol to reference the appropriate document.

  • Patient qualifies for automatic acceptance checkbox.

    • If the patient meets criteria to be admitted without a physician select this box.


Reason for Transfer

Reason for request to transfer.

  • Reason

    • Type to auto populate the reason.


Requested service

Type of medical service requested.

  • Specialty

    • Type to auto populate the specialty service within the service line.

  • Procedure

    • Document the requested procedure. This field does not auto populate.

  • The referring facility offers this service checkbox

    • Select if the Referring facility provides the same service requested.


Destination Details

Information about destination facility

  • Preferred Facility

    • Type to auto populate the preferred facility.

    • Select the information button to see more about the preferred facility.

    • When the case is saved this facility is populated in Facility Communication within the Communication tab. There is also a log saved in the Case Log.

  • Destination Facility

    • Type to auto populate the Destination facility where the patient will be transferred to.

    • Utilize the information button for more about the destination facility.

    • When the case is saved an event is added to the Case Log as well as the Facility Communication.


Case Type

  • Case Type

    • Type to Auto Populate the type of case.
