The final section within the Outcome Tab, Transport Request, focuses on information about getting the patient quickly to their destination.

We’ll cover Transport Details, Estimated time of arrival, Arrival Details, and contact details within this section.
Transport Details
Mode
Type and select the mode of transportation from a list.
Transport Company
Select the company from the dropdown.
Dispatch Date/Time
Enter the Date/Time for the transport from the Calendar tool or select Now.
Internal Transport Checkbox
Select this if transport modes internal to the destination facility have been offered.
Contact Details
Used to track contacts involved in communication with transport. Select Add an Additional to create up to four additional contacts.
Name
Type the Full name of Transport Service Staff
Phone
Type the phone number
Ext
Enter the Extension
ETA
Used to track the Estimated Time of Arrival for the patient.
Date/Time
Populate the Date/Time from the Calendar tool, or the current time by selecting Now.
Patient Arrival Details
Date/Time
Populate the Date/Time from the Calendar tool, or the current time by selecting Now.
Directed To
Type the specific directions given to transport staff as to where the patient was directed to.
