About

The Basic Information section contains much of the patient details/identifiers. One of the first sections populated during the transfer request.


Steps

We’ll step through each subsection to review what information can be entered.


Patient Details

Primary Information about the patient.

  • Last Name

    • Enter Patient Last Name. Required to create a case.

  • First name

    • Enter Patient First Name. Required to create a case.

  • Middle

    • Enter Middle Initial.

  • Suffix

    • Enter Patient suffix such as JR or SR.

  • DOB (Date of Birth)

    • Use the Calendar tool to enter the date.

  • Age

    • Age will Auto calculate based on the DOB.

  • Gender

    • Select Gender.

  • Patient Type

    • Type to populate from the list.


Home Address

In order to update home address information, click on the Add home address information link.

You’ll then be able to populate the below freehand:

  • Address

  • City

  • State

  • Zip


Patient Identifiers

Stores identifiers for registration. Can be entered freehand or using a lookup if the patient already exists in your system.

  • MRN

    • Enter Medical Record Number.

  • Visit Number

    • Enter Visit Number.

  • SSN

    • Enter Social Security Number.


Primary Care Physician

Stores information about the primary care physician.

  • Name

    • Type to populate the physicians name from the list, or search for a new physician.

  • Phone

    • Auto populated based on selected physician.

  • Ext

    • Auto populated based on selected physician.


Infection Prevention

Select Yes or No to designate if the patient has traveled outside the US within 21 days. Only appears to U.S. Setup.


Payor Information

To enter Payor information, click the Add Payor Information link. Begin typing in any field to populate the health insurance carrier responsibility to pay for the patients care. You can populate three payors:

  • Primary

  • Secondary

  • Tertiary
