Demographics, Vital Signs, and Transfer Reason

The addition of Demographics, Vital Signs, and Transfer Reason fields in Referral IQ® enables referring facilities to provide structured patient information during referral submission. This eliminates the need for free-text entry and manual follow-up by Transfer Center Agents, reducing errors and streamlining case processing.

How the Demographics, Vital Signs, and Transfer Reason Fields Work

Functionality

  • All new fields are displayed by default, but can be hidden as needed. To disable the default display, please contact TeleTracking Support.

  • Fields display exactly as they do in Transfer IQ and are linked to the same dictionaries.

  • Data entered in Referral IQ will populate directly into the Transfer IQ case form.

  • Updates from Transfer Center (TC) or CMS will overwrite Referral IQ entries through the feedback loop, ensuring data accuracy.

  • Fields can be edited by the referring facility until the case is imported into Transfer IQ.

  • Terminology for all new fields is internationalized.


Demographics

  • Demographics display in Referral IQ the same way as in Transfer IQ.

  • Information captured in this section flows directly into the Transfer IQ case.

  • Structured demographic entry reduces reliance on free-text notes and ensures consistency across systems.


Vital Signs

  • Vital Signs display in Referral IQ the same way as in Transfer IQ.

  • Users can enter up to five sets of vital signs per referral submission.

  • Data automatically transfers into the Transfer IQ case, minimizing the need for phone calls or manual transcription by Transfer Center Agents.


Benefits

Problem Addressed

Previously, external care providers could not capture structured demographic details, vital signs, or transfer reasons in Referral IQ. Instead, they relied on free-text notes or phone calls, creating extra work for Transfer Center Agents who had to re-enter this information into the appropriate case fields.

Solution Provided

With these new fields:

  • External care providers can submit structured information directly in Referral IQ.

  • Data automatically transfers into Transfer IQ, reducing redundancy.

  • Transfer Center Agents can validate entries instead of manually transcribing them, improving efficiency and reducing the risk of error.

Document Upload Related Changes

In order to support the ability for Document Uploads we’ve also brought Referral IQ®’s framework in-line with Transfer IQ®. With this comes some other functionality improvements.


Split Status & Disposition Columns

The single “Status” column is now split into two: Status (for Case Status) and Disposition. This change makes it easier to track a case’s progress and final outcome separately.

When a case is imported, the status will become In Progress, and when the case is completed, the status will be Complete.


Expanded Clinical Notes Functionality

We improved the Clinical Notes field , increasing the character limit from 255 to 3000 and supporting full edit history and inline edits—even after conversion to a case.

Before a referral is converted to a case, the Clinical Notes field is a simple textbox.

After conversion, it becomes a Case Log Note with full edit history. Both External Care Providers and Patient Placement Specialists can edit the note, and updates sync across systems with user and timestamp details shown.


Document Upload Enabled

A new Document Upload section lets users attach referral-related files directly in the referral form. Uploaded documents are synced with the corresponding case in Transfer IQ®, ensuring a centralized and secure process. Users can add or delete files at any time, and changes made in one view are reflected in the other.


Improved Date Filtering

Instead of a separate filter box, date filtering is now available directly within the Created Date and ETA columns, simplifying how users refine referral lists.


Withdraw Action Removed

The Withdraw button is no longer available. To remove an unneeded referral, users can import it as a case, apply a disposition, and mark it complete.


View Referral Button Removed

The previous “View-only” mode has been removed. Referrals now open in edit mode until they are converted to cases, at which point most fields become locked.
