Assessment Scoring Report Description

Permissions Required

You must have the Run Any Report, Including Those That Reveal Patient Information permission to generate this report.

Purpose

The Assessment Scoring report can help your organization to evaluate patient acuity and patient care needs. It is helpful to have the data in this report when you are determining improvements for process and care delivery. The report displays numeric score data from the PatientTracking Portal® list views about patients. Examples are Waterlow or Braden scores and Early Warning Sign (EWS) scores. Data is grouped by patient or by unit.

Report Parameters

Criteria selected when the report was being generated appear under Report Parameters. An example is the name of the campus selected when the report was generated.

About the Report Data

  • The user who generates the report can select specific units. Only data about patients with home locations in the selected units appears on the report. If no specific units were selected, all units in the selected campus can appear on the report.

  • The user who generates the report can select specific patients to appear on the report. If no specific patients were selected, then all patients who have home locations in the selected units appear on the report.

  • A specific "score type," such as Early Warning Sign (EWS), Braden, or Waterlow, is selected for the report. Only data about the selected score type appears on the report.

  • A date range and time period is selected when the report is generated. Only scores that were entered into a PatientTracking Portal® list view during that date range and time period appear on the report.

  • Data can be grouped by patient or by unit. If it is grouped by patient, then there is a section for each patient showing the patient's score, the date and time that the score was entered into a PatientTracking Portal® list view, and the unit with the patient's home location. If date is grouped by unit, then there is a section for each unit showing the patients with home locations in that unit, their scores, and the date and time that the score was entered into a PatientTracking Portal® list view.

Heading row

If data is grouped by patient, then the heading of each section shows the patient's name, medical record number, and visit number. If data is grouped by unit, then the heading row shows the unit name and each patient name with medical record number, and visit number listed underneath.

Date and Time

Date and time that the score was entered into the PatientTracking Portal® list view.

Unit

This column appears only if data is grouped by patient. It shows the unit where the patient's home location is. The report shows only patients with home locations in the units selected when the report was generated.

Score Type

This column header shows the name of the score type selected when the report was generated. This is the name of a column that can appear on a PatientTracking Portal® list view and has been designated as a score column in the Dictionary Management section of the Admin Tool component or Admin > Settings > Capacity Management. In earlier versions of the PatientTracking Portal® application, a score column could only contain a numeric value. A score column can now contain values in numeric, text, or date format. The administrator designates which columns can be score columns. Examples of scores are Early Warning Sign (EWS), Braden, and Waterlow. This column or the report shows the score for the patient whose name appears above the data.

First column

If data is grouped by patient, then the first column shows the patients' names. If the data is grouped by unit, then the first column shows the unit names.

Medical Record Number and Visit Number

These columns show each patient's medical record number, and visit number.

Date and Time

Date and time that the score was entered into the PatientTracking Portal® list view.

Patient

This column appears only if data is grouped by unit. It shows the patients' names.

Unit

This column appears only if data is grouped by patient. It shows the unit where the patient's home location is. The report shows only patients with home locations in the units selected when the report was generated.

Score Type

This column header shows the name of the score type selected when the report was generated. This is the name of a column that can appear on a PatientTracking Portal® list view and has been designated as a score column in the Dictionary Management section of the Admin Tool component or Admin > Settings > Capacity Management.
