Readmit a Discharged Patient - Capacity IQ®

This article will walk you through how a patient may become discharged and how to readmit them.

Patients may become discharged due to an incorrect discharge order or if another patient occupies the patient's home location within TeleTracking. Follow these steps to bring that patient back into TeleTracking.


Steps

Here a few possible ways to readmit the patient:

  • Send over a cancel discharge order. If a discharge was sent across, this can possibly repopulate the patient.

  • Transfer the discharged patient to a holding location and then transfer them back to their home location.

  • Send a readmit order against the patient. The patient will retain their visit number if needed and this prevents a mismatch between the client's ADT system and in CMS.

  • If the user has the group permission "Reoccupy Patient", they should be able to correct the patient from Capacity IQ® PreAdmit.

    • Search for the discharged patient visit and open the Patient Placement Details screen. From the Patient Action menu select Reoccupy Patient.
